New Patients
To expedite your registration in our office please print out and complete the Personal & Health Information Form. A completed form is required for each family member.
If you wish us to bill your insurance directly, you will be required to pay the remaining balance on the day of the appointment. In order for us to bill your insurance directly and to advise you of your coverage we will need to know your dental insurance group number, and your certificate number.
Please bring the following documents with you to your first appointment:
- Your Benefits Booklet from your employer or union, which includes your dental benefits.
- Your wallet Identification Card for dentistry (this is usually different from your drug identification card).
